I am aware of the nature of this activity and I hereby assume responsibility for my child's participation and to be photographed for publicity purposes. I waive and release HASTINGS ELITE SPORTS ASSOC., its Board members, coaches, volunteers, sponsors, and the facilities used for this program from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known, anticipated, or unanticipated, resulting from or arising out of my child's participation in this program. I further agree to indemnify and hold harmless HASTINGS ELITE SPORTS ASSOC. and all of its representatives from any and all liability for injury or illness sustained while participating in this program.
